In the《Shanghan Lun》tradition, this formula is described for a complex, hard-to-resolve pattern after vigorous purging in a cold disease: a sunken, slow wrist pulse with no pulse in the lower position, cold reversal of the hands and feet, a constricted throat, spitting of pus and blood, and unceasing diarrhoea — a picture the text itself calls difficult.
